Immunization Consent Required for all newly enrolled children and provides CPS the permission to share your child’s immunization records with Adventure Club.
Individualized Care Plan Required by any child that is administered prescription medications, has asthma, ADHD/ADD/ODD etc., and requires a signature by your child’s physician.
Medication Authorization This form provides Adventure Club with permission to administer medications while your child is in our care.
